Oral GLP-1 Pills: Foundayo, the Wegovy Pill and Compounded Alternatives

For a lot of people the injection is the barrier, not the medication. That has changed in 2026: there are now FDA-approved oral GLP-1s for weight management, and they are cheaper than the injectables at several providers. There are also compounded oral and sublingual products that look similar in marketing and are not remotely the same thing.

The three categories of oral GLP-1

Foundayo (orforglipron)

A once-daily oral GLP-1 approved for chronic weight management. Because it is a small molecule rather than a peptide, it can be taken at any time of day with no food or water restrictions.

The Wegovy pill (oral semaglutide)

An FDA-approved oral semaglutide for weight management. As a peptide it requires specific absorption conditions — follow the dosing instructions precisely or absorption drops substantially.

Rybelsus (oral semaglutide)

FDA-approved oral semaglutide for type 2 diabetes. Prescribing it for weight loss is off-label.

Compounded oral and sublingual

Drops, tablets and under-the-tongue formulations sold by telehealth platforms. Not FDA-approved, not studied to the same standard, and with no approved oral tirzepatide equivalent at all.

Why the peptide distinction matters

Semaglutide is a peptide, and peptides are broken down in the digestive tract. Getting oral semaglutide to work required a specialised absorption enhancer and strict dosing rules — take it on an empty stomach with no more than about 4 ounces of plain water, and wait before eating. Orforglipron is a non-peptide small molecule, which is why it escapes those constraints entirely. That is a genuine convenience advantage, not marketing.

What the trial data shows for Foundayo

Lilly reports that in trial data, average weight loss with Foundayo was approximately 7.4% to 11.1% at 72 weeks depending on dose, compared with about 2.1% for placebo, and that people taking the 17.2 mg dose lost up to about 25 pounds. In the ATTAIN-1 trial, adults on the highest dose lost an average of 27 pounds. Those are meaningful results, and they are also lower than the averages reported for injectable tirzepatide — a real trade-off between convenience and magnitude of effect.

There is no FDA-approved oral tirzepatide

If a provider offers oral or sublingual tirzepatide, it is a compounded product with no approved equivalent and no comparable evidence base. That is a considerably bigger leap than compounded injectable tirzepatide, which at least mirrors an approved product's route of administration.

The storage point deserves more weight than it usually gets. Oral GLP-1s remove refrigeration and cold-chain shipping from the equation entirely, which matters if you travel frequently, have unreliable delivery, or simply do not want to organise your life around a cold pack arriving on a weekday.

Frequently asked questions

Are oral GLP-1s as effective as injections?

Generally not as effective as injectable tirzepatide. Lilly reports average weight loss of approximately 7.4% to 11.1% at 72 weeks for Foundayo depending on dose, which is meaningful but below the averages reported for injectable tirzepatide. For many patients the convenience and adherence advantage outweighs the difference — that is a conversation to have with your prescriber.

Is the Wegovy pill the same as Rybelsus?

Both are oral semaglutide, but they are approved for different indications and at different doses. Rybelsus is approved for type 2 diabetes; the Wegovy pill is approved for chronic weight management. Using Rybelsus for weight loss is off-label.

Should I use compounded oral or sublingual semaglutide?

Approach it with more caution than compounded injectables. Peptides are poorly absorbed orally, which is why the approved oral semaglutide product required a specialised absorption enhancer and strict dosing rules. Compounded oral and sublingual formulations have not been through that development or review process. Ask any provider offering one what evidence supports its absorption claims.

No — oral tablets are stored at room temperature, which removes the cold-chain shipping and storage requirements that apply to injectable GLP-1s. If refrigeration or reliable delivery is a practical problem for you, this is a genuine advantage.
