What GLP-1 Medications Actually Cost in 2026

August 5, 2026·12 min read

Every GLP-1 provider advertises a number, and almost none of them is what you will pay. Membership fees, dose escalation and prepayment requirements move the real figure by hundreds of dollars a month. This guide shows how to calculate your actual twelve-month cost, using published prices from the providers we track.

The three things that break the advertised price

1. Membership fees

A platform advertising $99/month medication may add a $99-$150/month membership. Eden charges $99/month after the first month, Alloy $99/month, Maven $150/month. That can double your cost.

2. Dose escalation

Providers that price by dose start cheap and get expensive. Coby Health runs $239 at 2.5mg tirzepatide and $439 at 12.5mg. Rixa runs $280 to $525. Flat-rate providers hold the price.

3. Prepayment

The lowest per-month figures usually require paying for 3-12 months upfront. Cora's $99/month is $1,188 billed once. Fifty 410's $133/month is $399 billed once.

4. What's not included

Labs, shipping surcharges and cold-chain fees appear separately at some providers. Amazon states labs are not included; Midi has a separate refrigerated shipping charge on some products.

Brand-name GLP-1 pricing, 2026

Brand-name pricing splits into three very different tiers depending on how you access it, and the spread is enormous for identical medication.

RouteExample pricingNotes
Insurance copayFrom $25/mo (Zealthy, Maven, Amazon with coupon terms)Requires your plan to cover GLP-1s for weight management; prior authorisation usually needed
Manufacturer self-pay channelsFrom $149/mo (oral) to $349-$499/mo (injectable)LillyDirect and NovoCare referenced by Teladoc; Amazon lists $149/mo with coupon if eligible
Telehealth cash pricing$1,149-$1,695/mo (Levity); $1,399-$1,695/mo (Eden)Same FDA-approved drugs at several times the cost of the routes above
Do not pay four figures without checking

Levity charges $1,149-$1,599/month and Eden lists $1,399-$1,695/month for brand-name GLP-1s. Amazon One Medical lists the same category of medication from $149/month with a coupon. Before paying a four-figure cash price, check your insurance and the manufacturer self-pay programs.

Calculate your real twelve-month cost

  1. Identify your likely maintenance dose. For tirzepatide that is often 10-15mg weekly; for semaglutide, 1.7-2.4mg. Do not budget at the starting dose.
  2. Look up the provider's price at that dose, not its advertised entry price.
  3. Add twelve months of any membership fee.
  4. Add lab costs if the provider does not include them. Shapely publishes $75 flat at Labcorp; Amazon states labs are separate.
  5. Add any shipping or cold-chain surcharges.
  6. Check whether the low rate requires prepayment, and whether that prepayment is refundable if you stop.

Worked example: compounded tirzepatide for a year

ProviderStructureApprox. 12-month total
Cora Health (annual)$135/mo flat, all-inclusive, prepaid$1,620
Fifty 410Four prepaid 3-month programs at mid-dose~$2,000
Precision Telemed (maintenance)$179.99/mo flat, month-to-month~$2,160
Belle HealthFrom $175/mo, no subscription~$2,100+
Coby Health (monthly, titrating to 12.5mg)$239 rising to $439/mo~$4,400
Rixa Health (at 15mg)$525 per 4-week cycle~$6,800

Illustrative totals based on published pricing, assuming continuous treatment for twelve months at a therapeutic dose. Your actual cost depends on dose, plan and promotions.

The spread is the point

The same category of medication, taken for the same length of time, ranges from about $1,620 to about $6,800 depending only on which provider you chose and how they structure pricing. That is the difference careful shopping makes.

Is prepaying worth the discount?

Prepaying a year typically saves 40-45% versus month-to-month, which is real money. The counterweight is discontinuation risk: a meaningful proportion of patients stop GLP-1 therapy within the first few months because of side effects, cost or insufficient response. If this is your first GLP-1, the flexibility of a monthly or quarterly plan may be worth more than the discount. If you have taken one before and know you tolerate it, prepaying is usually the right call.

Read the refund policy before prepaying

"Cancel anytime" and "refund the unused portion of a prepaid year" are not the same promise. Coby Health states no refunds after shipment plus a $50 post-issuance fee. Fitish RX states all sales are final except verified errors. Fifty 410 ships the entire course upfront. Know the terms before you send four figures.

Ways to reduce your cost that people miss

  • Check your employer benefits. Programs like Teladoc and Calibrate are frequently employer-sponsored, which can make them free to you.
  • Check your own health system. Indigo Health charges $99 for a virtual visit with no membership fee and sends the prescription to your own pharmacy — many regional systems now offer something similar.
  • Use HSA or FSA funds. Most cash-pay GLP-1 charges are eligible with documentation, which is an effective discount equal to your marginal tax rate.
  • Ask about a maintenance rate. Precision Telemed charges $179.99/month for maintenance versus $249.99 for the full program; Coby's same-dose bundles drop to about $179/month.
  • Consider a lower dose deliberately. Microdosing programs from AgelessRx start at $99/month, though the evidence base is weaker than for standard dosing.
  • Recheck pricing every few months. This market moves fast, and switching providers is usually straightforward.

Frequently asked questions

What is the cheapest way to get a GLP-1 in 2026?

If your insurance covers GLP-1s for weight management, using it — copays can be as low as $25/month. If not, the cheapest verified all-inclusive cash option we found is Cora Health at $99/month for compounded semaglutide on an annual plan ($1,188 upfront), followed by Fifty 410's prepaid tirzepatide programs at $133/month.

Why do GLP-1 prices vary so much between providers?

Three reasons. Compounded product costs a fraction of brand-name to produce. Providers structure pricing differently — flat-rate versus dose-tiered versus membership-plus-medication. And some cash-pay telehealth platforms simply price at a large premium; Levity charges $1,149-$1,599/month for brand-name products available elsewhere for a fraction of that.

Do GLP-1 prices go up when my dose increases?

It depends on the provider, and it is the most important structural question to ask. Flat-rate providers like Cora Health, Precision Telemed and CoreAge Rx hold the price at every dose. Dose-tiered providers like Coby Health and Rixa Health increase it substantially — Coby runs $239 to $439, Rixa $280 to $525.

Are labs included in GLP-1 program pricing?

Usually not, and most providers do not say so clearly. Amazon One Medical states explicitly that labs are not included and must be purchased separately. Shapely publishes a $75 flat rate at Labcorp for uninsured patients. LevelsRx says its providers order labs but does not publish the cost. Ask before you enrol.

How much does brand-name Zepbound or Wegovy cost without insurance?

It depends entirely on the channel. Manufacturer self-pay programs have brought injectable brand-name pricing down substantially, and Amazon One Medical lists prescriptions from $149/month with a coupon if eligible. Unmanaged telehealth cash pricing for the same drugs runs $1,149-$1,695/month at some platforms. Always price the manufacturer channels first.
